I'm confused
I haven't taken mine apart yet so can't really tell - but, on the left of the picture - there's 2 possible places for an extra stud?

No, only one, the one at the top left. the bottom left is either broken or missing, can't tell from the photo.

And is there a thread in that hole? or does it need to be drilled and tapped or?

It's already threaded, just add a stud.

It's already threaded, just add a stud.
Welded cast steal is ok? I thought it was crap to weld?

Cast Iron is a PITA to weld but it can be done by someone who knows what they are doing. Trick is to find someone that knows what they're doing as opposed to someone who thinks they know what they are doing

Job complete!
I now have an eight studded manifold and a car that no longer sounds like a tractor. Manifold looks great - as if it came like that from factory.
The difference is noticable - the car is quieter to run, feels more responsive, accelerates from cold and seems to sit better at lower revs when cruising.
Doesn't feel any quicker when warmed up - in fact maybe the opposite. I wonder if the leak caused a flat spot somewhere or some extra lag? It certainly pulls nicer through the rev-range now though.
Costs for those looking to do it:
$200 Lug added to manifold & machining.
$ 60 Manifold gasket (facelift gasket is slightly more expensive).
$ 25 Studs & exhaust nuts.
$285 Total excluding labour
$355 Labour
$640 Total Cost
Happy with the price (I originally budgeted $800).
Hopefully the repair lasts the life of the car (which hopefully is quite some time). Off on a massive road trip next week so it'll get a good testing.
