Help finding this hubcap...

Hey guys,

Can anyone point me in the direction of where I can find one of these hubcaps? I'm assuming its a factory cap but I could be wrong on that.

Anyone know a model number or "name" of it?


Long story, but I need to find either one or a replacement set!
