Help... Again

So last week I was worried about my cert. Its now certed, but it doesn't go.

Took my head off on Wednesday (boy am i getting good at this) and found I have taken a large chunk out of cylinder 4's piston. Right on the edge. Thus, no compression.
I have just recently rebuilt this motor (with assistance from 'proper' engine builder) with semi-forged pistons.
On top of the four pistons are your regular four dimples. Where one of the dimples is at its thinnest is where the chunk has come off. Looking through the hole left over, I can see the ring, and the interlock or end of the ring is EXACTLY in the centre of the hole.
Has this possibly contributed to the chunk breaking off?
Is there a possibility that the ring seat has not been machined properly?
It has had chrome rings instead of factory ones, and I am unsure of the ins and outs of them, maybe some one here has had experience with them?

There is a slight possibility that it has been leaning out, although the temperature hasn't been high at all, it was running normal. No detonation either, and it didn't have the exhaust oxy sensor hooked up as such was blowing black smoke. Definately not blue, checked several times by different people following me.

I know its probably difficult to answer these questions, but if any one could help, that would be great.
