When you use full throttle, or idling, or in warm up it runs on its preset closed loop programs. the oxy sensor is only really there to satisfy emission laws in japan etc which is why most NZ new vehicles are not fitted with them, other than Oz spec cars.
As for the young ladies problem yes the cold start injector is a good place to start or the CTS.

Not to be fussy but you have put open and closed loop in the wrong places.
Closed loop is when the o2 sensor is being used, as the loop goes,
ECU injects the fuel into the engine
engine burns the fuel out to the exh
O2 read the exhaust and sends back to ecu
ECU injects the fuel into the engine
etc etc and the cycle continues.
Open loop the ECU accepts no feedback from the o2.
