On mine, the only things that ever caused this were:

Idle control valve stuck open
Vacuum leak

I replaced it twice, and they all crapped out, I ended up blanking the hole with a bit of 5mm ali and joining the hoses together.

Check your vac hoses first, the one I've had trouble with before goes to the steering rack.

there is NO power going to the Idle solenoid stepper motor thing in the throttle body - thats if they run the same type of set up as the 1JZ and 2JZ GE pre vvti. I know when i got my surf going i had a 2500rpm idle. put power to the idle motor and it closes and then you can control it from there. Also sounds like you have a vacuum leak or the TB is jacked open.

Yeah I'd say vaccum leak, I doubt the ISCV could flow enough air for a 4000rpm idle!

It's not as bad as you'd think, mine happily idles as soon as I start it, just have to watch it doesn't stall when you let off, but after a minute or 2 its fine. With mine, once I've made it from my driveway to the end of the road (maybe 200m) it will hold idle perfectly.
