Yeah, I found that same problem with a 2TG I rebuilt ages ago.. the previous owner had installed forged pistons, and regularly revved it to 9000+rpm.. without modifying the factory oiling system. It had actually shat a bearing, causing the crank journal to smash itself on number 3 conrod, and numbers 2 and 4 rod bearings were identical to yours!
You have to get the crank machined, as was mentioned. I just junked my block, and bought another one.. worked out to be much cheaper. Bad luck! Causes? Lack of oil, oil pressure, incorrect clearances..
