RE: OBLIGATION OF OWNER TO SUPPLY PARTICULARS OF DRIVER/PASSENGER IN RESPECT OF SECTION 118 OF THE LAND TRANSPORT ACT 1998

On Wednesday 21st December 2005 at about 12:10hrs the motor vehicle, registration number ####, which is registered to you, was involved in an offence on The Strand, at the intersection with Gladstone Road, Auckland City. The driver of the vehicle failed to wear a seat belt.
You are hereby given notice to supply such information in your posession or obtainable by you that will lead to the identification of the driver and / or passengers in your car at the time that the above incident occured.
It should be noted that failure to comply with the direction given under the provision of the above Act within 14 days on receipt of this Notice, makes you liable to a fine not exceeding $10000 in a Court of Law and possible disqualification from holding or obtaining a drivers license for such a period as the Court thinks fit.

All details must be supplied within fourteen days from the date of this letter.
On receipt of this information an infingement notice will be sent to the driver. This notice will show the amount of the fine and how it is to be paid.
If the driver's details are not supplied within fourteen days you, as the owner, will the incur a charge of Failing to Supply Information with a penalty of up to $10000
